Critical to a well-written report is that it meets the requirements of both format and content. When writing a report, connecting threads between thoughts must be present.
For example, issues, analysis, recommendations and implementation must flow from each other.
Ideally, all recommendation (s) will fall in line with the identified strategy.
The main recommendation(s) should be aimed at fixing the system, not just the symptoms.
The following section outlines the written and content requirements for a case report.
The written case report should follow these guidelines:
Audience: You will normally be writing your report to a specific person. Assume this person is already familiar with the facts of the case. Do not simply repeat the facts. Rather, use them as required, to support.
Style: Your report must be typed double-spaced. It must have at least 2.5cm margins on all edges; and, it must have a white background, complete with page numbers. Write in complete sentences. Do not use point form, expect when providing a coherent list in a wider context. Resist the temptation to use too many new “tools” (e.g. clip art, colour). Black-on-white is all that is required.
Font: The acceptable font is 12-point for the report and 10-point for exhibits.
Length: 1,500 words for each case study; this does not include the title page, table of contents, executive summary, or exhibits. You may have a maximum of three pages of exhibits.
Your report should contain:
Title Page: This is a separate page that contains the name of the case and the sate you submitted the report.
Table of Contents: This page lists the heading and page number of each section and helps the reader to navigate through the sections of the report.
Executive Summary: This is a one-page statement of the problem, the purpose of the communication and a summary of the results, conclusions, and recommendations. For a report to be considered complete it must contain an executive summary.
Body of the Report: This portion of the report analyses the data and answers the questions: “Why is there a problem?” And “What should be done to solve the problem?” It contains both the analysis and solution.
Conclusion: It emphasizes the message of the report.
Exhibits: Exhibits may be used for such things as drawing, process flow diagrams or showing detailed calculations. Remember that the report should stand alone; the exhibits provide supporting information only.
Format
Written Communication: Format, Language and Style: A report should contain titles, sentences and point form lists (if required). It should demonstrate a professional tone and as far as possible, be free of spelling and grammatical errors.
While format supports the flow of the report, the content forms the core of the report.
The content allows the writer to demonstrate his/her understanding of the issues and provide an in-depth analysis, recommendation, implementation and follow-up plan.
Overall Content
Operational Perspective: Your report should demonstrate your ability to synthesize the information, to make assumptions on the problem described in the case and to prove your understanding of the environment. The perspective is not a section on its own; it is demonstrated throughout the report and easily identified within the executive summary and detailed analysis, recommendation and implementation plan.
Judgment and integration: Pay close attention to the logic, structure and clarity of the analysis. Is there a connecting thread or a sequence in the analysis or is it a mix of ideas? Is your judgement sound and based on facts? How well have knowledge and concepts learned throughout the course been integrated?
Background
Logical Ship Management (LSM) is one of the oldest and also one the larger ship management companies in the world. It has eight offices in the various shipping hubs around the world, namely Shanghai, Singapore, Dubai, Hamburg, Rotterdam, Southampton, Miami and Houston. The company manages vessels for a variety of local small and large ship owners from its mentioned offices, which include tankers of various type and sizes, cape size and panamax bulk carriers, and container vessels ranging from 2,900 TEU to 10,000 TEU. The Miami office manages the cruise vessels. Very few of the vessels are “sister” vessels and most of them have been built in different shipyards in Asia and Europe.
The company is now trying to venture into a new market and negotiating contracts with a large local offshore company in Brazil for managing their fleet of offshore vessels and hopes to open an office there soon. Most of these offshore vessels are either DP2 or DP3 rated by a large classification society.
In each of its existing offices, the company manages an average of about 40 ships which are registered either in the port where the offices exist or one of the larger flag of convenience (FOC) like Panama and Liberia. The ratio between the local flag and FOC vessels is 1:3. The sailing staff and the office staff are all of mixed nationality, except where the local rules require having local crew and officers on board. The company has had a long-standing policy that in normal conditions none of the technical superintendents employed should manage more than 4 ships at a time and the fleet manager more than 20 vessels, thus for ease of understanding it can be assumed that there are two groups in each office. When one of the superintendents is on leave or attending dry-dock, the remaining vessels for that period are distributed among the superintendents in the office from the same group evenly for backup. The backup for the Fleet Managers is the Technical Director. The company’s manning agents along with their local training facilities exist in various parts of the world based on from where the majority of the crew and officers are employed. Their largest manning agent is from Manila. The rest of the functions like purchasing, HSEQ (Health, Safety, Environment and Quality) etc. are in-house and occupy the same offices as the technical team.
Based on the above background information, four cases are presented to you below. In each of the cases you will be asked, as a ship superintendent, to write a detailed professional report to the company’s Technical Director explaining the circumstances, giving details of your investigation and action taken, including the various agencies contacted and the advice given to the shipboard management team, if any.
Case Study 1
You are working for the Rotterdam office of LSM as a Ship Superintendent, and this week you have been under a little pressure because besides looking after the four LR2 tankers, you have been looking after a 4800 TEU container vessel too as a back-up for one of your colleagues who is on vacation.
It’s about noon time and you are getting ready for lunch when you get an urgent call from the master of one of the four LR tankers that you are managing that there has been an oil spill overboard due to the sudden facture of one of the pipes on deck while loading crude oil.
Write a detailed report to the senior management of LSM Rotterdam on your action on hearing this, whom you contacted, how you sorted out this problem and how you will ensure that it is avoided in the future etc.

FORMAT FOR ALL ASSIGNMENTS
The assignments are to be done in Microsoft Word with Times New Roman 12- point font. Each assignment should use the modified APA 6th edition format. The assignment templates are set up in this modified format. The reference page should be in the APA format for every reference. The word requirement for the assignments are specifically stated in the guidelines of the assignment. The word count is from the first word of the introductory paragraph to the last word of the conclusion paragraph. FYI.
REFERENCE REQUIREMENTS
1- There should be a cited reference for each section of a paper as there are conclusion statements in each section (to meet the grading criteria) so the conclusion should be supported by a reference as the assignments are evidence-based research, clinical papers.
2- Word Count- The word count is done from the first word of the introductory paragraph to the last word of the conclusion. The word count must be within the minimum and maximum word limit stated in the assignment instructions. A 10% – point reduction will be made in the total points for the assignment.
Week 1 and 5 assignments should have SIX studies. Week 5 assignment is a compilation of sections from the week 1, 2 & 3 assignments.
Weeks 2 and 3 need to have at least THREE studies in the form of research studies. Please make sure that you look at the instructions for the assignment as well as the grading rubric. I have also provided templates for ALL assignments. It is in your best interest to use these as it will guide you so that you do not make common formatting errors.
APA FORMAT FOR ASSIGNMENT
Please utilize the resources in the STUDENT SUCCESS CENTER if you have difficulty with APA 6th Edition format, grammar, sentence structure, punctuation or tense structure.
SUBMISSION OF ASSIGNMENTS
Always submit the assignment in the drop box for that assignment. I cannot accept any assignment as an attachment to an email sent to my college email. Attaching it to a posting to the Questions to Instructor Forum or the Individual Forum is also not accepted.
I cannot grade any assignment unless it is placed into the drop box by the student per university policy.
Turn-It-In / Lopes Write
This is to be used for the assignments for these three weeks (2, 3 & 5) and it is NOT required for Week 1. All assignments will be placed through Turn It In first and then submitted in the drop box on the course shell. No assignments will be accepted as attachments to emails as this is not the university policy.
Grading Rubric-
The rubric for each assignment is found in the upper right- hand corner of the assignment page under the Forum Tab. Compare the requirements of the rubric with your paper prior to submission into the drop box so you will receive full credit for all graded criteria of the paper.
Late Assignment-
Any assignment that is late will have 10% deducted each day it is late. This is a 5- week class. It is imperative that you ask for help if you are having difficulty with an assignment.
You need to contact me as soon as possible if an assignment will be late prior to the submission deadline.
